Care Coordination, Treatment & Crisis Services
Care Coordination
Care Coordination is a service to assist individuals who reside in a community setting or are transitioning to a community setting to gain access to needed medical, social, educational, and other services and supports, such as housing, transportation and entitlement programs. Services are provided to children and youth ages 5-18 and adults ages 18 and up. On-call is available 24 hours a day - 7 days a week.
Multisystemic Therapy (MST)
Multisystemic Therapy (MST) is an intensive family and community-based treatment that addresses the multiple determinants of serious antisocial behavior in juvenile offenders. The multisystemic approach views individuals as being nested within a complex network of interconnected systems that encompass individual, family, and extra familial (peer, school, neighborhood) factors. Intervention may be necessary in any one or a combination of these systems.
Crisis Services - Respite (Children and Youth)
Personalized Recovery Oriented Services (PROS)
Recovery-oriented program that assists individuals living with mental illness to achieve their life goals. The program integrates treatment, psychiatric rehabilitation, and vocational services to provide comprehensive supports to individuals in recovery. Individuals enrolled in the PROS choose goals that are meaningful to them and are assisted by PROS staff to develop the skills necessary to succeed in the life role of their choice. Services may be provided both on and off-site in order to best meet the needs of the individual, and may occur within a classroom setting or one-to-one.
Safe Options Support (SOS)
Combating Homelessness in New York
RSS provides intensive outreach, engagement and care coordination services to New Yorkers experiencing homelessness through its Safe Options Support (SOS) teams. Funded through the state Office of Mental Health, these outreach and referral-based teams work with affected individuals, typically in marginalized and underserved communities, to connect them to critical services and shelter.
RSS has a keen understanding of the magnitude of the homeless crisis, the trauma that homeless individuals experience, and the services best suited to transition someone from homelessness to permanent housing. Across upstate New York, RSS operates dozens of mental health housing programs, clinical services, and outreach services that aid individuals who have severe mental illness and may be homeless.
The mission of the RSS SOS teams is to provide voluntary, high-quality, culturally responsive, efficient, and cost-effective interventions grounded in the Critical Time Intervention (CTI) model. By promoting inclusion, listening, and applying a diverse approach to engagement, we assist individuals experiencing street homelessness and those in temporary shelter settings.
The RSS SOS teams consist of behavioral health professionals that include licensed clinicians, care managers and peer specialists. These outreach professionals provide immediate assistance, while also developing positive relationships that help individuals living on the street build life skills and strengthen their support network so their care can successfully be transferred to community-based providers and supports.
Through its SOS teams, RSS serves the following areas in upstate New York:

Housing Options
Treatment Apartment Program
OMH-licensed residential units of one, two, or three bedrooms with visiting staff who provide individualized Medicaid-reimbursed services similar to, but generally less intensive than, those offered in a group living setting. Residents are expected to transition to more independent living when able. On-call is available 24 hours a day - 7 days a week.
Pleasant Street Residence
Mental health adult (18 and over) community residences are OMH-licensed group homes that vary in size from 8 to 14 individuals. They serve Medicaid-eligible individuals needing 24 hours a day - 7 days a week care from staff, who work intensely with the residents in formulating individualized service plans. These generally include medication management and training, life skills training, and services and interventions that support health and wellness. Group living programs are intended to transition residents to more independent settings as soon as possible. (Programs specializing in serving individuals dually-diagnosed with mental illness and chemical abuse are indicated with the acronym MICA, those serving exclusively geriatric residents with the abbreviation GERI, those providing short-term (less than 6 months) transitional living are indicated with the acronym STCR, those serving individuals with mental illness and developmental disabilities are indicated with the acronym MIDD).
Revonah Hill Residence (MICA)

Employment
Supported Employment Program
Supported employment assists individuals to secure competitive employment in jobs best suited to their abilities and preferences. Ongoing staff and informal supports are available to help with job retention. Supported employment may include ACCESS –VR funded intensive services consisting of job development, placement, situational assessment, job coaching, and OMH-funded extended services.
Wellness
Children & Youth Recreation Program
The goal of this after school, out-of-home program is to teach the youthful participants socialization and recreational skills while giving their parents/guardians respite.
