Care Coordinator

Job Description
Responsibilities include care coordination for 12-20 children, youth and families using wrap around services through the Orange County System of Care.  Provides community and home visits and assists with following through on their wrap around plan.  Works closely with other System of Care team members.  Minimum qualifications include: Master’s Degree in Social Work, Psychology or related field and two (2) years experience preferably with children, youth and families or Bachelors degree and four (4) years experience with children.  Bilingual skills (English/Spanish) preferred.  Criminal background check and DSS registry clearance required.

The candidate should hold a current New York State license in Social Work or Mental Health Counseling. The qualified candidate will be responsible for duties including, but not limited to: providing face to face clinical services to clients for 85% of the working day, providing group-based psychotherapy, facilitating family work and linkage with community providers, and accurate and timely completion of all related documentation to OMH and agency standards. The candidate will be proficient in collaborative documentation and evidence based clinical practice. Additionally, the qualified candidate will be able to work as a member of an interdisciplinary treatment team. Proficiency in word processing and web-based applications are a must, as collaborative documentation in the electronic record is a large part of the job. Rehabilitation Support Services, Inc. is seeking a Full-time Master’s level clinician to work with the juvenile justice population in our Mental Health Juvenile Justice Program in Orange County. This program works within the Department of Probation. The candidate will be responsible for weekly therapy sessions, conducting mental health assessments, and reporting progress to the court.

Master’s degree required; clinical supervision hours available. Social Workers, Mental Health counselors and bilingual Spanish-speaking candidates are encouraged to apply.

An opening is available for a Case Coordinator within a Supported Housing/Certified Apartment Program. A Case Coordinator assists individuals who have a mental health diagnosis to obtain and maintain housing as well as learn new skills to become more independent. The qualified candidate will be responsible for duties including, but not limited to medication supervision, provision of rehabilitative services, coordination with landlords, money and benefit management, and timely completion of all documentation. Additional responsibilities include on-call hours. High School Diploma or equivalent required in addition to a minimum of one-year experience in Human Services. A criminal background check, driver's license, and a reliable vehicle are also required.

Rehabilitation Practitioner - PROS

Job Description
An opening exists in the RSS Capital District PROS Program for a Rehabilitation Practitioner.  The PROS program is a person-centered, recovery-oriented model that serves adults recovering from mental illness.  Qualified candidates must have a Bachelors degree, however preference will be given to candidates with Masters degrees.  The candidate should have a minimum of one 1 year experience providing direct service relating to the care or treatment of persons with a designated mental illness.  The qualified candidate will be responsible for duties including, but not limited to the provision of classroom/group services, the provision of one to one services, and the accurate and timely completion of all documentation.  An understanding of the PROS model and the principles of psychiatric rehabilitation are a plus.  Experience assisting individuals living with mental illness in securing and sustaining employment is also a plus.  The position may require flexible hours, including evenings and weekends.  Completion of criminal background check required.
