Rehabilitation Support Services, Inc. . is seeking full time Care Managers for our community-based Care Management Program located in our Poughkeepsie, NY office. Our Care Managers are responsible for the coordination of services to individuals living throughout Dutchess and Ulster counties who may be experiencing chronic medical, mental health, and substance abuse needs. 17. Ensure audits of case records are conducted according to division protocol, if
applicable.
