The goal of the program is to provide community stabilization. Reliable transportation required. Rehabilitation Support Services, Inc. . is seeking full time Care Managers for our community-based Care Management Program located in our Poughkeepsie, NY office. Our Care Managers are responsible for the coordination of services to individuals living throughout Dutchess and Ulster counties who may be experiencing chronic medical, mental health, and substance abuse needs. This is not a site-based position but, is one where you will be working with people living in the community and assisting with accessing needed services and benefits to ensure they achieve their goals and lead healthy, independent lives. We provide client centered care that is sensitive to the autonomy of the people we serve. Our program provides a stimulating work environment where creativity and independent problem solving are encouraged. Position will require an ability to provide comprehensive coordination of all health care needs including medical, mental health, substance abuse, social services, and wellness. Although our care managers are part of a team, the ideal candidate should be comfortable working independently in the community. An ability to be organized is very helpful. You should have a desire to make a difference and provide quality care to a vulnerable and often underserved population. An ability to work with high risk/high needs individuals in a fast paced and challenging position with complex needs that will include: mental health, substance abuse, and chronic health conditions.

Minimum Qualifications
An associate’s degree in one of the following fields:
social work, psychology, rehabilitation, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field.

Active/valid driver’s license

Successful completion of the Justice Center and New York State Office of Children and Families Statewide Central Register Database clearance.
