Rehabilitation Support Services, Inc. . is seeking full time Care Managers for our community-based Care Management Program located in our Poughkeepsie, NY office. Our Care Managers are responsible for the coordination of services to individuals living throughout Dutchess and Ulster counties who may be experiencing chronic medical, mental health, and substance abuse needs. This is not a site-based position but, is one where you will be working with people living in the community and assisting with accessing needed services and benefits to ensure they achieve their goals and lead healthy, independent lives. We provide client centered care that is sensitive to the autonomy of the people we serve. Our program provides a stimulating work environment where creativity and independent problem solving are encouraged. Position will require an ability to provide comprehensive coordination of all health care needs including medical, mental health, substance abuse, social services, and wellness. Although our care managers are part of a team, the ideal candidate should be comfortable working independently in the community. An ability to be organized is very helpful. You should have a desire to make a difference and provide quality care to a vulnerable and often underserved population. An ability to work with high risk/high needs individuals in a fast paced and challenging position with complex needs that will include: mental health, substance abuse, and chronic health conditions.

Minimum Qualifications
An associate’s degree in one of the following fields:
social work, psychology, rehabilitation, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field.

Active/valid driver’s license

Successful completion of the Justice Center and New York State Office of Children and Families Statewide Central Register Database clearance.

Bi-lingual a plus

Experience is a plus

Job Title: Assistant Manager
Hours of Work: Full-time, 40 hours per week. Non-Exempt.
Minimum Qualification: Bachelor’s Degree in human services or Associates Degree in human
services with three years’ experience in human services preferred or a high school degree and
five years experience preferred in human services.
Source of Supervision: Program Manager/Program Coordinator/Program Director
Essential Functions:
1. Supervise staff.
2. Assist Program Manager in an orientation plan that is developed and implemented for
new employees.
3. Maintain a positive work environment that contributes to team building.
4. Conduct regular staff meetings if applicable.
5. Ensure all policies and procedures are carried out.
6. Oversee client care, including crisis services and assure all services are provided on a
timely and regular basis.
7. Ensure all applicable medication regimes are carried out.
8. Oversee the coordination of all needed recovery oriented client services.
9. Attend client related meetings as needed.
10. Ensure the protection of all client civil liberties, rights and properties.
11. Advocate client need with other service providers.
12. Conduct regular client meetings and encourage client participation in program
planning and decision making.
13. Work cooperatively with all internal and external entities.
14. Ensure premises are well maintained, professional and meet all safety requirements.
15. Assure that all billing is accurate and complete according to Agency policy,
Medicaid, or other regulatory guidelines if applicable.
16. Ensure all required documentation and record keeping is completed within time
frames according to Agency policy and regulatory requirements.
17. Ensure audits of case records are conducted according to division protocol, if
applicable.
18. On-call responsibilities if applicable.
19. Ensure that all staff maintain appropriate certifications and attend all required
trainings.
Other Duties:
1. Participate in training sessions designed to enhance growth and skill development.
2. Performs other duties as assigned by the immediate supervisor or other management
staff.
3. Maintain valid CPR, First Aide and Defensive Driving certification if applicable
