Transition Support Team

The Transition Support Team serves individuals who have experienced long-term stays in the Psychiatric Center or state operated community residences.  The Transition Support Team (TST) can work with individuals for up to 9 months prior to their discharge from the hospital or state operated community residences in order to engage with the individual and assist them in preparing for the transition into a community-based setting.  This is accomplished by exploring community resources and assisting individuals to apply for benefits, housing, treatment and rehabilitation programs so that they can immediately access these resources upon discharge. Additionally, the TST provides on-going supports for individuals for up to 6 months after they have been discharged from a psychiatric center or state operated community residences.
