Home and Community Based Services (HCBS) Schenectady County

Home and Community Based Services (HCBS) provide opportunities for Medicaid beneficiaries with behavioral health conditions to receive services in their own home or community.

Managed Care Organization (MCO) HARP’s manage the enhanced benefit package of Home and Community Based Services. HARP’s use HCBS to provide person centered recovery services to eligible people with mental illness and/or substance use disorders.

Eligibility Requirements include age 18 or over, the individual is enrolled in a HARP Plan, and placement in the program is based upon a documented medical necessity approved by the Managed Care Organization (MCO).
HCBS Services depend on which county you reside: The following services are offered in Schenectady County: Psychosocial Rehabilitation, Habilitation, Education Support, Pre-Vocational Services, Intensive Employment, Ongoing Supported Employment.

Supported Counties
