Capital District Stabilization and Support (CDSS)

Program provides access to 3 dedicated beds at South Lake Community Residence.  Guests are referred to avoid psychiatric hospitalization and/or emergency room visits and are provided with a safe therapeutic environment to work on wellness and stabilization.  Guests must have existing housing, however staff assist with making referrals to other community resources including Health Home Care Management.

Services provided include: Assertiveness/Self Advocacy, Community Integration/Resource Development, Rehabilitation Counseling, Daily Living Skills, Health Services, Medication Management, Parenting Training, Skill Development, Socialization, Substance Abuse Services, and System Management.  An Individual must be 18 years old or older, be diagnosed with a severe and persistent mental illness (SPMI) who is not a imminent threat to self or others and  has a desire to engage in the service.
