Working in our Case Management Program, position involves linking, coordinating, monitoring, and advocating for services for individuals in need of ongoing mental health support in order to maintain or enhance living in their community. Two years of experience in providing direct services to people who are mentally disabled or homeless or Associate’s Degree and 1 year of case management experience required as well as a valid NYS driver’s license and successful completion of criminal background check. Spanish speaking a plus.
