Position works within a Care Management Program which serves individuals who present with complex and varied health care issues. The Sr. Care Manager is responsible for working with a high volume of individuals across all health care domains of psychiatric, physical and addiction disorders. The Sr. Care Manager will coordinate all areas of care to ensure that people reduce their hospitalizations. The Sr. Care Manager will actively collaborate with consumers and providers to prevent hospitalizations by engaging them in community/outpatient services and supports. The Sr. Care Manager is highly organized, can manage his/her time effectively and has strong written, verbal and computer skills. May provide some staff/program supervision. This position requires travel. The minimum requirements are a Bachelor’s Degree with a concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community health, child or family studies, sociology, speech or hearing. Four years experience in providing direct services and/or linking clients to a broad range of services essential to successful living in a community setting. A master’s degree in a related field may be substituted for two years of the required experience.
