Participates in the intake and screening of new clients and as a team member in individual, group and family therapy. Provides social work services to assist clients to adjust to their disabilities and social environment. Coordinates activities and treatment programs with other agencies and involved others. Collects and maintains data from various sources in order to establish an overall approach to assist clients to improve/maintain their level of functioning. Provide care coordinator or case managements services, as applicable. May provide services to families and clients in their homes. Master’s degree in Social Work from an accredited school of Social Work. Criminal background check and valid NYS driver’s license are also required.
