Registered Professional Nurse-HOP/Medical Enhancement

An opening exists in our Housing Options Plus/ Medical Enhancement program for a Registered Professional Nurse.  Housing Options Plus / Medical Enhancement is a community based housing program specifically designed for individuals with chronic medical needs in addition to their mental health diagnosis. Qualified candidates must be licensed as a registered professional nurse by the New York State Education Department and should have experience providing direct service relating to the care or treatment of persons with a designated mental illness. The qualified candidate will be responsible for duties including, but not limited to conducting health assessments, developing care plans, coordinating services with individuals’ medical and psychiatric providers, medication supervision and medical monitoring  in the community, provision of rehabilitative services, provision of health and wellness education for consumers and staff, and the accurate and timely completion of all documentation.   Valid NYS Driver’s License and completion of criminal background check required.
