Position works with children and adults who carry severe and persistent mental illness.  Responsibilities include: Intakes and assessments, assist client with achieving goals maximizing their independence and productivity through support, training and assistance in use of personal and community resources; coordinates and participates in the development of individualized goal oriented service plan; provide outreach to clients who are in need of mental health services; participate in regular SPOA meetings, on-call responsibilities, etc. Computer experience is required.  Minimum qualifications: Bachelor’s Degree in Social Work, Psychology, Nursing, Education or related field and four years’ experience in providing direct services to persons with mental disabilities and linking the client to a broad range of services essential to successfully living in the community OR a Master’s Degree in related field with two years of required experience.  Competitive salary and benefits package.  Position also requires a valid NYS Driver’s License and a background check.
