Housing Specialist- HOP / Medical Enhancement

An opening exists in our Housing Options Plus Supported Housing / Medical Enhancement program for a Housing Specialist. Housing Options Plus / Medical Enhancement is a community based housing program specifically designed for individuals with chronic medical needs in addition to their mental health diagnosis.  A Housing Specialist assists individuals who have serious mental health diagnosis to obtain and maintain housing as well as learn new skills to be successful in the community. In addition, this position will assist individuals with chronic medical needs to successfully maintain their housing. The qualified candidate will be responsible for duties including, but not limited to medication supervision, provision of rehabilitative services, coordination with landlords, money and benefit management, and timely completion of all documentation. High School Diploma or equivalent required in addition to a minimum of one-year experience in Human Services. A criminal background check, a driver’s license and a reliable vehicle are also required.
