The RSS Otsego Care Management Program has a full-time position available for a Health Home Plus Care Manager. The position is Monday-Friday 8-4:30, with some flexibility. The primary role of the Care Manager is to provide intensive Health Home services to a caseload of 15 individuals with the goal of reducing the rate of hospitalizations through engagement in all necessary community based resources and services, and behavioral and physical health care. The Care Manager will work closely with community providers to ensure appropriate delivery of services to meet the needs of each participant. The Care Manager will conduct various assessments to assist in identifying client needs and create individualized care plans to reflect these needs. Care Managers must be extremely organized and demonstrate an ability to carry out multiple ongoing tasks with the ability to prioritize. This person will be able to work independently and is highly dependable. The candidate will also have strong written, verbal and computer skills.
Minimum qualifications are a Bachelor’s Degree in Human Services or a related field or a CASAC along with 2 years of experience in the Human Services field.
