Job Title: Community Gatekeeper
Hours of Work: Full-time, 40 hours per week. Non-Exempt.
Minimum Qualifications: Graduation from a regionally accredited or New York State registered college or university with a bachelor’s degree in human services and at least one year of experience providing direct services within behavioral health care.
Source of Supervision: Program Director via Delaware Counties Department of Mental Health, Director of Community Services.

Essential Functions:
1. Engages clients and conducts phone and risk assessments to identify and prioritize an
individual’s needs.
2. Ability to determine level of crisis presentation and make timely referrals to appropriate resources.
3. Prioritizes and directs scheduling of mental health clinic intake appointments.
4. Familiarity with available community resources.
5. Provides referrals to peer services and other appropriate community resources.
6. Exhibits effective engagement and outreach skills.
7. Protect and ensure confidentiality and client rights.
8. Ability to navigate an electronic record.
9. Ensures protection of all client civil liberties, rights and property.
10. Prepares reports and maintains records.
11. Maintains an active and valid driver’s license.
12. Participates in supervisory meetings.

1. Performs other reasonably related duties assigned by the immediate supervisor or other management staff
as required.
