Case Manager – HOP/Medical Enhancement

An opening exists in our Housing Options Plus Supported Housing / Medical Enhancement program for a Case Manager. Responsibilities include assessment of physical and behavioral health needs, medical care coordination, assisting individuals in following through on health care recommendations, health/wellness promotion, advocacy and support in medical appointments, collaboration with health care providers, medication supervision and monitoring, provision of rehabilitative services, crisis intervention and timely completion of documentation. Bachelor’s degree with experience preferred. A criminal background check, a driver’s license and a reliable vehicle are also required
