An opening is available for a Case Coordinator within a Supported Housing/Certified Apartment Program. A Case Coordinator assists individuals who have a mental health diagnosis to obtain and maintain housing as well as learn new skills to become more independent. The qualified candidate will be responsible for duties including, but not limited to medication supervision, provision of rehabilitative services, coordination with landlords, money and benefit management, and timely completion of all documentation. Additional responsibilities include on-call hours. High School Diploma or equivalent required in addition to a minimum of one-year experience in Human Services. A criminal background check, driver’s license, and a reliable vehicle are also required.
