RSS is currently seeking a full-time dedicated and compassionate Care Manager/Service Coordinator to join our team in the Safe Options Support (SOS) Program.   As a Care Manager, your role will involve coordinating the needs of individuals transitioning from the streets to their new homes. You will work closely with participants, enhancing their daily living skills, providing accompaniment to appointments, and advocating on their behalf to ensure they receive appropriate care.

We prioritize member choice, harm reduction, non-coercion, flexibility, and person-centered care. As a Care Manager, you will play a vital role in delivering care that aligns with our program model and values. This is a field-based position that may require periodic on-call coverage and flexibility in working hours.

To be considered for this position, you must have a Bachelor’s degree or higher in a related field, or be a New York State Licensed Practical Nurse (LPN). Previous case management experience is preferred but not required.

Responsibilities of the Care Manager include conducting outreach and engaging with homeless individuals, assessing their health and social needs, assisting with hospital discharge planning, resolving clinical issues impacting their ability to manage supportive housing, and fostering relationships with community providers. You will also provide appointment navigation and accompany participants to appointments, conduct comprehensive psychosocial assessments, monitor their progress, and attend team meetings and supervisory sessions.

To excel in this role, you should have knowledge of homeless resources in the Southern Tier, counseling principles and methods for mental illness and substance use disorders, and familiarity with treatment, rehabilitation, and community support programs. Strong computer skills and documentation abilities are also essential. The position will require travel throughout Chenango, Broome, and Otsego counties.
