Working as a Case/Care Manager for the RSS Targeted Case Management Program, responsibilities include assessment of physical and behavioral health needs, care coordination, health promotion, linkage and referrals to necessary resources and services, advocacy and collaboration with health care providers, medication monitoring, and crisis intervention and documentation. Bachelor’s degree with experience or high school diploma/GED and 4 years human services experience required. Consideration will be given for social workers and LPN’s. Criminal background check and valid NYS driver’s license also required.
