Invisible Children’s Program
This program serves eight families, where one or both parents have a mental illness, in supported housing with case management services. The goal of the program is family preservation.
Sullivan MST
Multisystemic Therapy (MST) is an intensive family and community-based treatment that addresses the multiple determinants of serious antisocial behavior in juvenile offenders. The multisystemic approach views individuals as being nested within a complex network of interconnected systems that encompass individual, family, and extra familial (peer, school, neighborhood) factors. Intervention may be necessary in any one or a combination of these systems.
Supportive Apartment Program
OMH-licensed residential units of one, two, or three bedrooms with visiting staff who provide individualized Medicaid-reimbursed services similar to, but generally less intensive than, those offered in a group living setting. Residents are expected to transition to more independent living when able. 24/7 on-call is available.
Supported Housing Program
Non-licensed apartment living with rental assistance and case management housing support services provided to help individuals secure and maintain successful community living. Residents choose their own apartment, receive funding for furnishings and security deposits, and may choose to live alone or have the option of selecting an apartment-mate(s). Staff visit residents at least once a month or more often if necessary. 24/7 on-call is available.
Pleasant Street Residence
Mental health adult (18 and over) community residences are OMH-licensed group homes that vary in size from 8 to 14 individuals. They serve Medicaid-eligible individuals needing 24/7 care from staff, who work intensely with the residents in formulating individualized service plans. These generally include medication management and training, life skills training, and services and interventions that support health and wellness. Group living programs are intended to transition residents to more independent settings as soon as possible. (Programs specializing in serving individuals dually-diagnosed with mental illness and chemical abuse are indicated with the acronym MICA, those serving exclusively geriatric residents with the abbreviation GERI, and those providing short-term (less than 6 months) transitional living are indicated with the acronym STCR).
Revonah Hill Residence
Mental health adult (18 and over) community residences are OMH-licensed group homes that vary in size from 8 to 14 individuals. They serve Medicaid-eligible individuals needing 24/7 care from staff, who work intensely with the residents in formulating individualized service plans. These generally include medication management and training, life skills training, and services and interventions that support health and wellness. Group living programs are intended to transition residents to more independent settings as soon as possible. Supported Employment Program
Supported employment assists individuals to rapidly secure competitive employment in jobs best suited to their abilities and preferences. Ongoing staff and informal supports are available to help with job retention. Supported employment may include VESID-funded intensive services consisting of job development, placement, situational assessment, job coaching, and OMH-funded extended services.
Children and Youth Recreation Program
The goal of this after school, out-of-home program is to teach the youthful participants socialization and recreational skills while giving their parents/guardians respite.
Case Management
Case management is a service to assist individuals who reside in a community setting or are transitioning to a community setting to gain access to needed medical, social, educational, and other services and supports, such as housing, transportation and entitlement programs. 24/7 on-call is available.
